1. Select the provider you wish to see.
2. Select preferred location:
   • “Medical Blvd” located at 1660 Medical Blvd, Ste #100 or #300. Mon-Fri. 
   • “Cypress Way” located at 90 Cypress Way, Ste #40. Mon-Thurs.
   • ** If you don’t see openings, change the location to see more options.
3. Select date and time. (If you need to be seen sooner, call the office or send a patient portal message.)
4. Enter your personal information and the reason for your visit.
5. You will receive a confirmation email.
