A notification will be sent to you between 24 and 48 hours before your appointment. Please answer these questions in advance. The more you can complete at home, the faster you can see the provider.
Please arrive 30 minutes before your appointment to complete any remaining paperwork. If you are more than 10 minutes late to the scheduled appointment, you may need to wait for the next available time, or the provider may need to reschedule the appointment for another day. Same day cancellations with less than 24 hour notice and not arriving to a scheduled appointment will incur a $25 fee.
